Job Scope: Provide strategic leadership and direction for community care services. Oversee service delivery, quality, governance, and operational performance. Lead and develop multidisciplinary teams, supporting workforce planning and capability building. Build partnerships with healthcare, social service, government, and community stakeholders. Strengthen integrated care pathways and community engagement initiatives. Manage departmental budgets, compliance, risk management, and reporting requirements.
