Nurse Manager | Community Care Services

To lead the nursing team and related healthcare workers in the ward, and be responsible for the day-to-day operational work flow in the Nursing Department. Lead, supervise, and mentor a team of nurses and healthcare staff across community care services. Oversee daily clinical operations to ensure safe, efficient, and high-quality nursing care delivery. Develop, implement, and review nursing care protocols, workflows, and quality improvement initiatives. Collaborate with management and stakeholders on community healthcare projects, outreach programmes, and service development initiatives. Ensure compliance with MOH regulations, SNB standards, infection control policies, and workplace safety requirements. Manage manpower planning, staff scheduling, training, and performance evaluations. Support case management and multidisciplinary care planning for clients and residents. Monitor clinical indicators, incident reporting, and operational performance metrics. Build strong partnerships with hospitals, community agencies, caregivers, and external healthcare providers. Participate in strategic planning, audits, and organisational development initiatives.
