About this role
Perform comprehensive and client-specific speech and language assessments duringadmission, regular care reviews, and discharge planning. Utilize suitable outcome measures to monitor therapy effectiveness and inform timely clinical decisions. Collaboratively develop personalized and SMART (Specific, Measurable, Achievable, Relevant, Time-bound) therapy goals with clients and key stakeholders. Plan and deliver individualized, evidence-based speech and language interventions that prioritize client needs. Accurately document assessments, therapy plans, goals, interventions, and outcome measures in line with clinical guidelines. Communicate findings, treatment plans, progress, and discharge strategies clearly to clients and relevant parties. Create discharge plans and strategies to support continued care, developed in partnership with the care team and client. Promote successful integration into the community by working with relevant external agencies when necessary. Recommend and prescribe assistive devices and communication aids tailored to each client’s needs. Engage in multidisciplinary discussions to ensure holistic and coordinated client care. Maintain accurate, timely documentation and records that reflect therapy provided and client outcomes. Contribute to the planning, implementation, and evaluation of clinical programs and services. Provide support in mentoring and supervising Therapy Assistants. Actively participate in ongoing professional development activities. Involve in quality improvement projects and/or clinical research initiatives. Comply with audit standards and internal regulatory requirements. Undertake other duties as directed by the reporting officer.
