About this role
About Community Mental Health Department: Our Community Resource, Engagement and Support (CREST) teams support clients who are at risk of or diagnosed with mental illness as well as their caregivers. The teams provide basic emotional support and monitoring of their mental health conditions through home visits and service linkages. Responsibilities: Engage and collaborate with stakeholders such as government agencies, grassroots, and community-based organizations to raise awareness of mental health Conduct mental health screening to identify those who are at-risk/diagnosed with mental health conditions and connect them with the right services for early diagnosis and treatment Provide basic emotional and cognitive support to clients with mental health issues and/or their caregivers and link them up with relevant services based on their needs Ensure proper maintenance of case documentation and timely submission of reports to external parties Manage volunteers to support outreach events and conduct activities when necessary Establish and support the run of a caregiver support group Requirements: Degree or Post-Graduate Diploma in Social Work, Counselling, Psychology, or Degree or Diploma in Occupational Therapy or Nursing, or Degree in Clinical Mental Health Preferably 2 years of healthcare/community care clinical experience Passionate to work in the mental health sector and community services Proficient in Mandarin or dialects as the outreach talks may be required to be conducted in mandarin Excellent communication skills and interpersonal skills A team player who can also work independently Detail-oriented in keeping proper documentation This position is on a contract basis Work Service Area: Bukit Panjang SMC, Cashew & Zhenghua Brickland & Choa Chu Kang Limbang & Yew Tee Work Office Location : Teck Whye If you are interested to join us, please apply via Fei Yue Career Portal https://careers.fycs.org/ For your privacy and security, kindly refrain from including your NRIC number in your application form or resume.
