Case Manager (Youth Department - Youth Go! Programme)

Responsibilities: Conduct regular day and night street outreach, community walks and activities to engage street youths Provide counselling and case management for street youths and their families Design, implement and evaluate outreach activities and intervention programmes for street youths Plan, conduct and manage case conferences with community partners Document and maintain records of all outreach activities and casework sessions. Conduct talks and workshops in schools Adhere to professional ethics, organisational policies, and boundaries.
