Care Coordinator (Healthcare | West) #Hjo

Job Responsibilities: Support the care team in coordinating patient care and follow-up activities Build and maintain positive relationships with patients, caregivers, healthcare professionals, and community partners Assist patients in accessing healthcare and community resources to support their health and wellbeing Perform basic health screening activities and parameter measurements as required Provide basic health education on lifestyle management, chronic disease prevention, and healthy living practices Educate patients on the use of health monitoring devices such as blood pressure monitors, glucose monitors, and weighing scales Support care coordination activities aimed at improving patient outcomes and reducing hospital admissions Assist with social prescribing initiatives and community engagement programmes Maintain accurate records and ensure timely documentation of patient interactions and interventions Perform other duties and projects assigned by supervisor Requirements: Minimum O Levels, A Levels, NITEC, or Diploma qualification Customer service experience is preferred Strong communication and interpersonal skills Pleasant personality with a passion for patient care and community engagement Interested candidates are invited to email your resume to: jen.ooi@recruitexpress.com.sg Jenalyn Ooi Huai Sian (R1765551) Recruit Express Pte Ltd | Company Reg.
