Location Bedok – 1 opening Simei – 1 opening About the Role Join a patient-centred healthcare team within a community hospital environment, where you will play a key role in supporting patients through safe and effective medication management. You will work closely with multidisciplinary teams to provide quality pharmaceutical care, patient education, and support for rehabilitation and transitional care services. Key Responsibilities Dispense medications accurately and ensure safe pharmaceutical practices for inpatient and outpatient services Provide medication counselling and education to patients and caregivers to promote adherence and safe medication use Support medication reconciliation and chronic disease management initiatives Collaborate with doctors, nurses, and allied health professionals to ensure continuity of patient care Monitor and review medication therapy plans to optimise patient outcomes Maintain proper documentation and ensure compliance with regulatory and hospital pharmacy standards Assist in inventory management, procurement, and stock control activities Participate in quality improvement initiatives and departmental projects Support weekend and public holiday pharmacy operations when required Requirements Degree or postgraduate qualification in Pharmacy Registered with the Singapore Pharmacy Council Strong interpersonal and communication skills with a patient-centric approach Passionate about community and rehabilitative healthcare services Able to work independently as well as collaboratively within a multidisciplinary team Positive attitude with strong organisational and problem-solving skills ⚡ Immediate Hiring – Apply Now If you’re ready to level up your Pharmacist career and work that truly matters , don’t wait. 📩 Apply today or email your updated resume to sam@recruitco.com.sg (Only shortlisted candidates will be notified.)
