SAMH manages the Mobile Support Team (MST), it is a community based allied health-led team which provides psycho-social therapeutic interventions for clients with mental health needs and support for their caregivers to maintain client's wellbeing in the community. Overall job objectives Responsible for the provision of case management services and support for persons in recovery from mental health conditions/possible mental health conditions, and their caregivers. Core responsibilities Case Management Process referrals, conduct intake assessments and formulate individualised case plans to support clients’ recovery. Implement and coordinate follow-ups with case plans. Perform the necessary screening and assessment tools (eg. GAF, Zarit Burden) to facilitate case planning. Conduct home visits to clients and caregivers where necessary. To provide follow-ups and service linkages for clients in the community who are without support and not linked to services. Provide individual and family counselling, group work and support as part of case management in the following areas: Provide psycho-education to clients and caregivers on mental health issues and illness management. Educate clients and caregivers on medication management and coping with side effects. Educate clients on budgeting and financial management. Support clients in employment and work-related matters. Support clients in their self-care, activities of daily living and independent living skills. Conflict resolution and mediation. Empower clients in interpersonal/family relationships and communications. Assist clients with accommodation issues. Discharge planning. Collaborate with medical professionals and other social service agencies in supporting clients and caregivers for a holistic case management approach. Conduct regular case conferences and case discussions with other agencies. Manage behavioural and mental health concerns presented by clients. Identify early symptoms of relapse, provide feedback to psychiatrists and assist caregivers in arranging for clients’ admission to hospital for in-patient treatment. Educate and empower clients to advocate for their own recovery by working in collaboration with the medical team. Engage police and emergency services for assistance to handle crisis situations and violent behavior where necessary. To establish positive networking relationships and collaborations with community partners and funders. Administrative Support To write case recordings, case reports and correspondence to agencies. To gather client-related information and compile updated records of clients’ particulars for monthly and quarterly reports to meet funders’ statistical requirements. To assess clients’ eligibility for assistance applications and to do referrals where necessary. To assist in taking minutes during staff meetings and case conferences. Outreach Involvement To orientate and host visitors to SAMH centres. To conduct in-house and external talks, man information booths to promote SAMH services and to raise awareness of mental health concerns. To sit in work-related committees in collaboration with other community agencies. Other Matters To collaborate with and support other SAMH programmes or projects when required. Assist in coverage of duties when required. To be on stand-by when alerted to deal with sudden crises. Any other duties and official tasks as required by Supervisor and/or Reporting Officer.
