Healthcare Industry Reputable Organisation Competitive Remuneration Package As a Care Coordinator (Healthcare), you will be responsible for the following duties: Collaborate with the multidisciplinary care team to deliver holistic, patient-centred care. Empower patients and caregivers to manage their health through the use of digital health technologies and tools. Promote awareness and access to community and healthcare resources to support positive health outcomes. Build and maintain strong relationships with patients, caregivers, and internal stakeholders. Support preventive health initiatives by encouraging health screening and vaccination uptake. Collect and document patients' social determinants of health to support care planning and interventions. Coordinate and support patient care activities using IT systems for documentation, tracking, and monitoring clinical indicators. Perform basic health assessments and assist in preventive care programmes. Provide health education on lifestyle management, self-care, and physical and social well-being. Educate patients on the proper use of home health monitoring devices, including blood pressure monitors, glucometers, and weighing scales. Advise patients on recommended vaccinations and national health initiatives. Perform any other ad hoc duties as assigned. Requirements: Min.
