About this role
Primary Objectives of Position: To provide planning, coordination and evaluation of programmes to meet the needs of vulnerable families living in the shelter with the goal of strengthening families’ coping and resilience, strengthening family life, and maximizing their potential to lead dignified and independent lives. Job Responsibilities: Intake •Handle day-to-day general enquiries on shelter admission and related shelter matters •Manage referrals and conduct assessment of applications for admission into TS Casework and Counselling •Support assessment, planning and intervention when needed •Engage clients in a professional and therapeutic manner to understand needs, and to build rapport with clients including all members of the family (especially children and vulnerable family members) •Document and evaluate case outcomes •Identify, develop and review appropriate intervention strategies, in consultation with supervisor/centre manager •Organise, coordinate and conduct case conferences with external agencies Coordinate and monitor delivery of services with other community agencies •Assist with shelter visits and support in assessment of clients •Coordinate and support in documentation and evaluation of case outcomes Programme Planning, Coordination and Execution •Identify and assess clients who may benefit from group work/community programmes •Identify and support development of new group work/community programmes, evaluate and modify •Monitor and evaluate effectiveness of group work/community programmes periodically, including the group work and the impact on clients •Lead or assist in the implementation of respective types of group work (lead psycho-education groups that aim to provide information and/or assist in therapeutic groups) / community programmes Post-Discharged Support •Support post-discharge cases when needed •Assist in assessing, developing and implementing discharge planning for clients •Follow up with and monitor post-discharge status and/or outcomes of clients, and liaise with agency/ies on progress of clients •Assist in liaising with community agencies, and support planning and implementation of programmes and activities •Maintain daily records, update case files of clients and prepare reports for administrative and intervention purposes
