About this role
Conduct comprehensive and accurate physiotherapy assessments during client admission, periodic reviews, and at discharge. Choose relevant and specific outcome measures to assess therapy progress effectively, facilitating timely clinical decisions. Collaborate with clients and involved parties to establish individualized, SMART (Specific, Measurable, Achievable, Relevant, Time-bound) goals. Plan and deliver physiotherapy treatments that are person-centered and grounded in evidence-based practice. Maintain clear and thorough documentation of assessments, treatment plans, interventions, goals, and outcomes in accordance with established protocols. Share assessment findings, treatment strategies, goals, progress, and discharge plans with clients and stakeholders. Oversee the safe and effective use of rehabilitation tools and technologies and provide training on assistive devices where appropriate. Assess and recommend suitable assistive devices and equipment based on client needs. Develop discharge plans and strategies for continuity of care in collaboration with relevant team members, based on client performance. Promote integration into the community by coordinating with external community partners when necessary. Equip clients and caregivers with self-management techniques to support long-term health, mobility, and function. Take part in multidisciplinary case discussions to ensure coordinated and collaborative care planning. Ensure timely and accurate documentation and reporting of client care, outcomes, and relevant statistics. Contribute to the implementation, review, and development of clinical programs and services. Support the training, guidance, and supervision of Therapy Assistants. Engage in continuous learning and professional development activities. Participate in quality improvement efforts and research initiatives where applicable. Adhere to audit standards and regulatory requirements. Perform additional responsibilities as assigned by the Reporting Officer. Requirements: Minimum a Diploma or Degree in Physiotherapy Dynamic and proactive personality with good public relation and communications skills. Possess good management, excellent analytical and problem-solving skills.
