Work Location: Kent Ridge Job Responsibilities: Review written/electronic order form (medication chart, medication record or prescription) to ensure drug prescribed is appropriate. Perform medication reconciliation. Provide drug information service to health care professionals. Involvement in clinical services such as attending multidisciplinary rounds and running ambulatory services. Dispenses and counsel patient on medication prescribed by doctor. Assigns, oversees and closely review the work of pharmacy assistants and technicians. Training pharmacists to be advance generalists specialists. Craft out necessary learning objectives, curriculum, materials. Requirements: At least 5 years' relevant experience as Clinical Pharmacist with inpatient setting experience PharmD or equivalent degree recognized by the Singapore Pharmacy Council Experience in training and development of clinical curriculum/training programs Strong leadership and communication skills Interested candidates may whatsapp your resume to 8977 7062 .
