RESPONSIBILITIES ACCOUNTABILITY Reporting to: Social Work Assistant Manager, Social Work under the direction of Head of Home MAIN PURPOSE / OBJECTIVES: As a member of the Social Services Team, you are expected to provide effective Case Work Services for clients and support to provide high quality of care to meet the diverse needs of Children entering the residential service. To provide and ensure that the emotional, social, behavioural, developmental, therapeutic and educational needs of clients are met during the client's stay in the Home. You will need to support the Manager in guiding and ensuring work delivery in the Service and observance of Standard' of Care, Social Work practice and ethos. MAIN DUTIES AND RESPONSIBILITIES Case Work Management / Supervision 1.1 Effective use of therapeutic approaches and other intervention strategies to stabilize and regulate emotions of clients. Provide regular updates and work with other relevant professional to facilitatedecision making on placement plan for clients. Ensure behaviour support management strategies are in place to effectively manage challenging behaviours of clients. Support in developing and review of Standard Operating Procedures (SOPs) in compliance with the Ministry and Licensing guidelines. Provide training to direct care staff on communication techniques and other skills that will facilitatepositive interactions with the clients. Ensure that client admissions are facilitatedupon referral and a smooth transition for clients to settle down in the programme. To develop and regularly review the Individualised Care Plan of each client, with inputs from the Block Management Team. To work with the clients' families to facilitatefor a smooth transition and reintegration home. Programme Development and Evaluation Plan and implement programmes that will complement the goals of the Service and that are engaging and therapeutic in intervening with the children and young persons. Monitor to maintaina daily programme schedule to ensure that clients are kept sufficiently engaged. Evaluate the outcome of the project/Program against the objectiveson a regular basis. Community Resources and networking Establish close working partnership both internally (The Haven, Gracehaven, IPAC and Residential program Team and volunteers) and other stake holders so as tomaximise community resources and support systems. Service Development Advocate and represent the rights, needsand welfare of Clients through official platforms, to address service gaps, concerns or issues that will promote and enhance the service delivery for beneficiaries. Provide regular updates to the Social Work Manager and Senior Management (SM) and MSF on the progress of the clients and highlight Operational issues where relevant. Professional Development Promote professional development through identifying learning needs, transfer of skills and knowledge and offer training where appropriate. Administration Complete all necessary documentation for all clients including admissionforms, discharge summaries and other ongoing records. Support in staff induction/orientation/ appraisal/training when required. Coordinate and compile reports for presentation in various boards both internal and external. Undertake any other duties as and when requiredand in consistent with the position. QUALIFICATIONS Core Qualifications & Requirements Education: Bachelor's or Master's Degree in Social Work, or a Graduate Diploma in Social Work from a recognized institution.
