About this role

At 4PM, we work closely with youths and families to strengthen resilience, well-being and positive outcomes. We are expanding our initiatives and seeking a dedicated Social Worker to drive our structured mentoring and youth development programmes. This role combines programme delivery with casework and psychosocial support, working closely with volunteer mentors and community partners to provide timely, coordinated support for youths with diverse needs. Key Responsibilities: Implement and coordinate structured mentoring and youth development programmes for youths, including planning and running engagement sessions and group mentoring activities aligned to programme objectives. Collaborate with internal teams and external stakeholders to strengthen holistic support for youths, ensure safeguarding, and facilitate timely referrals when needed. Supervise volunteer mentors to ensure safe, meaningful and effective mentoring relationships, including mentor ongoing guidance. Provide casework management and psychosocial support to youths/beneficiaries with varying needs and presenting issues, including basic assessment, individual check-ins, brief interventions and appropriate referrals for higher-risk cases. Monitor and evaluate participant engagement, attendance, progress and outcome indicators using programme tools/systems, and contribute to continuous programme improvement and reporting. Maintain accurate documentation, case notes, reports and programme records, including support for grant compliance and reporting requirements. Requirements: Degree in Social Work. Minimum 1 year relevant experience in the social service sector Experience in programme delivery and group facilitation; comfortable working in community-based settings. Strong stakeholder management skills Good documentation discipline, confidentiality mindset, and ability to work independently while collaborating in a team.
