Job Title : Social Work Coordinator Key Responsibilities : Case Management : Independently manage routine cases assigned by MSWs, ensuring that patients receive appropriate care and support. Financial Assessments : Conduct assessments to determine patients' eligibility for various financial assistance schemes, ensuring timely and accurate processing. Field Visits & Discharge Coordination : Perform home visits and accompany patients to social service organizations as needed. Support the discharge process by facilitating a seamless transition for patients back into the community or home care. Training and Support : Provide guidance and training to new social work staff, including administrative assistants, to ensure effective service delivery and team collaboration. Patient Follow-Up : Handle follow-up work for patients post-discharge, ensuring continuity of care through communication with patients and community services. Networking & Collaboration : Work closely with community services, social service organizations, and other healthcare professionals to ensure patients receive holistic care and support. Job Requirements : Education : Diploma in Social Work, Counseling, Psychology, or a related field. Work Experience : Prior experience in social work or healthcare setting preferred, with exposure to case management and patient support services.
