Speech Therapist

Responsibilities: Conduct accurate and holistic speech and language specific assessment for clients during admission, periodic care plan reviews and discharge Select appropriate and specific outcome measures to enable evaluation of therapy progress and outcomes in a timely manner to guide clinical decision making Formulate person centered and SMART goals in collaboration with clients and relevant stakeholders Design plans and conduct speech and language interventions for clients in a person centered manner, that is also evidence based Document assessment results, therapy plans, interventions, goals and outcomes measures according to guidelines Communicate assessment results, intervention plans, goals, outcomes and discharge plans to clients and relevant stakeholders Formulate discharge and continuity of care plans based on clients’ performance in consultation with relevant stakeholders Facilitate community integration through collaboration with other community partners as required Prescribe assistive devices and equipment according to clients’ needs Requirements: Minimum Degree in Speech & Language Therapy and must be AHPCregistered Minimum 2 years of relevant experience Good team player and able to work independently Please submit your updated resume in Word format by using the Apply Now Button.
