Responsibilities: Conduct accurate and holistic physiotherapy specific assessment for clients during admission, periodic care plan reviews and discharge Select appropriate and specific outcome measures to enable evaluation of therapy progress and outcomes in a timely manner to guide clinical decision making Formulate person centered and SMART goals in collaboration with clients and relevant stakeholders Design plans and conduct physiotherapy interventions for clients in a person centered manner, that is also evidence based Document assessment results, therapy plans, interventions, goals and outcomes measures according to guidelines Communicate assessment results, intervention plans, goals, outcomes and discharge plans to clients and relevant stakeholders Ensure proper use of equipment including technological-assisted rehabilitation equipment Formulate discharge and continuity of care plans based on clients’ performance in consultation with relevant stakeholders Facilitate community integration through collaboration with other community partners as required Empower clients and caregivers with methods to enhance health, mobility and functions through self-management strategies outcomes.
