Community Care Assistants

Job Description: Establish and implement Individual Care Plan (ICP) for clients, including appropriate intervention plans. Assist in identifying clients who may show early onset of medical conditions. Conduct and facilitate group and individual activities, ensuring holistic care for clients. Guide and assist clients in daily activities such as exercises, games, handicrafts and outings. Assist with transportation and escort clients to, from and within the centre. Serve meals and provide support with personal hygiene care. Measure, record, and monitor clients' vital signs, including blood pressure, heart rate, pulse, and respiratory rate. Requirements: Prior experience in eldercare, healthcare, or social service sector preferred. Able to perform basic caregiving tasks, including personal hygiene care and meal assistance. Good communication and interpersonal skills to interact effectively with clients, caregivers, and colleagues. Patient, compassionate, and committed to enhancing clients’ well-being. Physically fit to assist clients in mobility and daily activities.
