Your Role As a Social Worker/Case Manager, you will play a key role in supporting residents through comprehensive case management and care coordination. You will work closely with residents, multidisciplinary teams, families, and community partners to promote residents' well-being and independence. Key Responsibilities Conduct comprehensive assessments of residents' physical, psychological, social, and financial needs. Develop, implement, and regularly review Individual Care Plans (ICP) to support residents' rehabilitation and well-being. Coordinate care with healthcare professionals, therapists, social workers, and community agencies to ensure holistic support. Provide supportive counselling to residents and deliver psychoeducation to enhance their understanding of mental health conditions, treatment, coping strategies, and recovery. Advocate for residents' rights and facilitate effective communication between residents, families, and the multidisciplinary team. Plan, organise, and evaluate programmes that promote residents' rehabilitation, social integration, and quality of life. Respond appropriately to crisis situations and provide timely intervention when required. Maintain accurate case documentation and ensure compliance with organisational policies and regulatory requirements. What We're Looking For Diploma or Degree in Social Work, Psychology, Counselling, Nursing, or a related discipline. Relevant experience in case management, healthcare, social services, or community care is preferred. Strong interpersonal, communication, and problem-solving skills. Compassionate, organised, and able to work effectively within a multidisciplinary team. Proficient in documentation and basic computer applications. Why Join Us?
