Patient Service Officer (Primary Care Coordinator)

The Role Working location: 80 Bendemeer Working hrs: Mon-Fri office hrs The Primary Care Coordinator supports the implementation and administration of Primary Care Network (PCN) and Healthier SG (HSG) programmes. The role is responsible for programme reporting, compliance monitoring, service coordination, and operational support to ensure participating clinics meet programme requirements and performance targets. Key accountabilities Prepare, collate, and validate programme data for reporting, submissions, and compliance tracking. Prepare administrative reports required by MOH/AIC for programme funding and performance reporting. Coordinate PCN ancillary services such as Diabetic Retinal Photography (DRP), Diabetic Foot Screening (DFS), Nurse Counselling, and Spirometry. Monitor programme performance and support clinics in meeting PCN and HSG requirements. Support the implementation of PCN and HSG initiatives and drive continuous improvement efforts. Maintain accurate records and documentation to support audits, funding claims, and governance requirements. Qualifications & Experience Diploma or Degree in Healthcare Administration, Public Health, Life Sciences, Healthcare, or a related field. Preferably 1–3 years of experience in healthcare administration, programme coordination, operations, or project support. Experience in data reporting and stakeholder coordination is advantageous. Familiarity with PCN, Healthier SG, or Singapore healthcare programmes is an added advantage
