Job Purpose: To support the Medical Social Work Department in providing emotional, social, and practical assistance to residents and their next-of-kin through referral management, administrative support, enquiry handling, basic supportive counselling, and coordination of social and financial assistance services, with the aim of enhancing coping, well-being, and quality of life. Details and Responsibilities: Receive, process, and maintain referral records for nursing home and day care centre admissions Support Medical Social Workers (MSWs) in intake assessments, admission coordination, and service arrangements Assist residents with community-related matters, including accompanying them to banks, Housing & Development Board (HDB), and other agencies as required for case management purposes Maintain accurate records and documentation to facilitate periodic reporting, statistical returns, audits, and ad hoc information requests Support the daily operations and administrative functions of the Medical Social Work Department Provide case management support to MSWs, including social-emotional support and practical assistance to residents, clients, and their next-of-kin Process applications for subsidies, means testing, and various financial assistance schemes Conduct financial assistance applications and renewals for residents and clients of the nursing home and day care centres Attend to public enquiries regarding nursing home and day care services, providing information, guidance, and appropriate referrals Collaborate with internal and external stakeholders to ensure smooth service delivery and continuity of care Perform any other duties and responsibilities assigned by Management and/or the Medical Social Worker Qualification, Education, and Skills: Minimum GCE 'A' Level / Diploma in Social Services or equivalent for those without experiences Minimum GCE 'O' Level or equivalent for those with at least 1-2 years in social service sector. Core Competencies: Good interpersonal and communication skills Ability to work with elderly residents and their families Strong administrative, documentation, and organizational skills Ability to handle sensitive and confidential information professionally Team player with a service-oriented mindset Proficiency in Microsoft Office applications Working Conditions: Required to accompany residents to banks, Housing & Development Board (HDB), government agencies, healthcare institutions, and residents' homes/properties as part of case management and social work support services Required to work with residents and next-of-kin who may present with complex psychosocial, behavioural, emotional, or family-related challenges May be required to work occasional evenings, weekends, or public holidays to support operational needs, resident welfare, or departmental activities
