Key Responsibilities: Attend daily interdisciplinary rounds to collaborate closely with physicians, nurses, and other healthcare professionals to develop and implement patient care plans in a home-based hospital setting Conduct daily virtual ward rounds with the patients, driving innovation in pharmacological care and ensuring optimal patient outcomes.
