About this role

As a Health Coach, you will be the crucial link between our patients’ treatment plans and their daily lives. Think of this role as part health architect, part concierge: you hold the full picture of each patient’s wellbeing, coordinate every element of their care — from TCM and wellness therapies to Western medicine and specialist referrals — and ensure nothing falls through the cracks. You will inspire, motivate, and educate patients, helping them navigate their health journey with confidence. This role is perfect for a proactive individual who is passionate about both holistic health principles and innovative wellness solutions. Job Responsibilities 1. Client Health Coaching, Education & Empowerment Coach and empower patients to become active, informed participants in their own health journey. Conduct personalised one-on-one health coaching sessions, assessing individual patient needs and tailoring strategies to support varying levels of adherence and motivation. Coach patients on foundational holistic health principles, integrating both Western and TCM approaches to nutrition, sleep hygiene, and stress management techniques. Provide evidence-based health information and resources, sharing national guidelines and recommendations where appropriate. Empower and motivate clients to make sustainable behavioural changes aligned with their lifestyle, health goals, and treatment plans (TCM and modern wellness technology therapies). Provide continuous support, encouragement, and guidance throughout the coaching process. Serve as a dedicated point of contact via our Health Coaching WhatsApp line, providing timely support for patient queries, encouragement, and motivation during office hours. Utilise coaching frameworks like SMART (Specific, Measurable, Achievable, Relevant, and Time-bound) goals to help patients build actionable, step-by-step plans for success, and provide strategies to overcome adherence challenges. 2. Care Coordination, Integrated Care & Referrals Act as the patient’s care concierge — mapping their full treatment landscape and ensuring every element is coordinated seamlessly. Maintain a clear, up-to-date picture of each patient’s active treatments across TCM, wellness technology therapies, and any external care, ensuring all elements are aligned with their overall goals. Proactively manage care continuity: track follow-up milestones, flag gaps between treatment phases, and ensure no patient loses momentum between appointments. Foster a strong, collaborative relationship with our TCM Physicians and clinic teams to ensure integrated patient care and seamless referral pathways. Identify and facilitate appropriate referrals — to Western medicine practitioners, specialists, or allied health professionals — ensuring patients receive comprehensive care for needs that arise beyond the clinic’s core treatment scope. Follow through on referral outcomes: monitor progress, communicate updates to the treating team, and incorporate learnings back into the patient’s care plan. Champion patient success by gathering and showcasing testimonials and feedback to continuously enhance service quality and demonstrate outcomes. Partner with multi-functional teams (e.g. medical, marketing) to author write-ups and guides on relevant wellness topics spanning TCM, Western medicine, and integrative wellness. 3. Clinic Ambassadorship & Operations Support Represent the clinic’s values and capabilities at every patient interaction, and contribute to a seamless clinic experience. Act as ambassador for the clinic’s suite of technology-enhanced services, educating clients on their benefits and integrating them into clients’ wellness plans. Manage patient inquiries with professionalism, empathy, and care. Maintain meticulous and confidential patient records, ensuring proper documentation of coaching sessions, clinic interactions, and progress notes. Proactively collaborate with clinic and medical teams to ensure a positive experience for every patient, and contribute beyond defined responsibilities when needed — including supporting operational flow and administrative duties as required.
