Our established public healthcare client in the west is looking for an Assistant Care Coordinator to join their growing team! 5 days office hours Supportive and stable work environment within the public healthcare sector Annual AWS + PB (up to 4 months) Responsibilities: Facilitate patient selection, counselling, enrolment and referrals to primary care partners. Provide training and develop workflows to assist hospital departments in right-siting efforts. Support continuous patient care within the community. Monitor and manage the transition of patients from polyclinics to general practitioner partners. Input and maintain accurate data in the tracking database. Engage patients in health promotion activities and national initiatives as needed. Execute additional tasks as assigned by supervisors and the Head of Department (HOD). Requirements: Diploma in a health-related discipline preferred. Service-oriented, proactive, and adaptable in a fast-paced environment. Ability to multi-task and work autonomously as well as collaboratively. Respond effectively to inquiries or complaints from doctors, staff, and patients. Proficient in Microsoft Office applications.
