Social Work Associate, Senior Service

RESPONSIBILITIES As a Social Work Associate within the Senior Services team, you will work to promote social-health wellbeing of seniors by engaging them meaningfully. Role & Responsibilities: Handle case related enquiry from clients and the community. Conduct regular home visits to monitor the well-being of clients. Assist social workers in casework assessment and interventions. Collaborate with external healthcare partners to coordinate seamless care. Maintain accurate and organized records and files. Handle administrative tasks, including data collection and report preparation. Participate in internal and external training, supervision and team meetings. Carry out any other duties as assigned by HOD and supervisor. QUALIFICATIONS Preferred Polytechnic Diploma in Social Work; other diplomas can be considered. Passionate to work with seniors and their caregivers in community setting. Ability to work independently and as part of a team. Strong communication and organizational skills. Effective written and oral communication in English. Fluency in Mother Tongue language and/or dialects will be advantageous. Prior experience in social service &/or healthcare setting preferred.
