Senior/Occupational Therapist - Office Hours - 5 Days (Palliative Care)

Manage clinical caseload and support palliative rehabilitation practice (e.g. breathlessness management, prescription of equipment, collaborative goal setting) across the various care settings. Deliver evidence-based individualised care through holistic comprehensive assessment, treatment and evaluation of care. To work collaboratively with multi-disciplinary colleagues to meet patient needs. Participate in service review to drive service improvement. Contribute to the implementation and monitoring of adherence to relevant clinical guidelines. To be a resource and support to other team members and community professionals. Take initiative in pursuing life-long learning and personal development through reflective practice and engage actively with learning and development opportunities Supervise and assign appropriate treatment plans to therapy assistants. Maintain high quality patient documentation and records. Any other duties as and when assigned.
